HeavenlyCare.247 ("we", "us", "our") is committed to protecting the privacy of participants, families, carers and visitors. This policy explains what personal information we collect, how we use it, and your rights under the Australian Privacy Act 1988 and the Australian Privacy Principles (APPs).

Information we collect

We may collect: name, contact details, date of birth, NDIS number and plan details, health and disability information, emergency contacts, cultural and language preferences, and notes relevant to providing safe and appropriate support.

How we use information

We use information to: assess referrals, deliver agreed supports, coordinate with your support network (with consent), meet our obligations as an NDIS provider, respond to enquiries, and improve our services. We do not sell personal information.

Disclosure

We may share information with your nominated support coordinator, plan manager, family members, treating health professionals, or regulators (NDIS Quality and Safeguards Commission) where required by law or with your consent.

Storage and security

Records are stored securely in Australia where practical. We apply access controls and staff confidentiality obligations to protect your information.

Your rights

You may request access to or correction of the information we hold about you, withdraw consent, or make a privacy complaint by contacting us at heavenlycare.247@gmail.comor 0432 041 509. Unresolved complaints may be referred to the Office of the Australian Information Commissioner (oaic.gov.au).
